So his name was only credited in 12 games? (7 for Ys I and 5 for Ys II) That's kinda low.

Well, he's credited in all the original Falcom versions of Ys I and II. Those are also the only games he were involved with, so he got 100% credit.
As for the licensed versions of YsI/II, like for PC Engine, Famicom, Sega, etc. he's not credited anywhere. I don't really know why.

Well now, when you put it that way it makes sense...
sorta.
I think the real question in this thread is this: "Why did he not get creditted in the other versions? Is he not the one who created the original tunes in the games? If so, why not at least mention him?"
